P-4389 Using the new MDS MPAF form.

 NOTES

The new MDS Medicare PPS Assessment Form (MPAF) can be used by your facility for the following MDS 2.0 PPS record types:

AA8a = 0, 5 or 10 with AA8b = 1, 2, 3, 4, 5, 7 or 8 

Note: All MPAF forms should be entered into the CHARTS system using CHARTS menu 13, MPAF - PPS Short Form. 

The following fields are on the MPAF form and are not on the RUGs III 1997 Quarterly form:

A5ba-f, D1, G5b, M3, Q1a and Q1c

These fields, if left blank, will not generate any state errors and are therefore also ignored by the CHARTS missing field report.

To enter a Quarterly form choose the correct RHS menu as follows:

For a non-PPS Quarterly (AA8a = 5, AA8b = blank) use CHARTS menu 5.

For a PPS Quarterly (AA8a = 5 and AA8b = 4) and you are not using the new MPAF form use CHARTS menu 12.

For a PPS Quarterly (AA8a = 5 and AA8b = 4) and you are using the new MPAF form use CHARTS menu 13.

Set-up

To activate the new MPAF form do the following:

From any CHARTS menu type NHOP2 or [GM, 17, 13] and press ENTER.

The following screen appears:

 At Option 39, MDS 2.0 - MPAF type Y as shown above and press ENTER.
