This patch contains a revised UB92 billing program to comply with new Medicare "Therapy Plan Established" edits.

Medicare recently introduced additional edits that require the "Therapy Plan Established" date on all Medicare Part B bills. This update will add the appropriate code to the Medicare bills and warn the user if the code is missing.

The Plan Established date for all therapy charges being billed must be updated.

As in the past the Onset dates and First Treatment dates must also be entered.
